Minnesota law () requires you to provide 911爆料网 with the month, day, and year of your immunizations on an immunization form. If you see the registration error message 'Immunization info required for registration', you will need to submit an immunization form.
Submit an immunization form
To meet the immunization requirements, please complete one of the following options (do not send copies of medical records):
- Online form: (processing time 3-5 business days)
- Alternate format: Immunization form (PDF)
This form must be printed, signed, and submitted to the Student Services Center (T.2100) or submitted to the as a PDF attachment. - International Student Immunization form (PDF)
- Paper forms are available at the Student Services Center (T.2100)
