Post-9/11 Transition and Care Management Program
Transition assistance to all Veterans who have served in combat since September 11, 2001. The program can provide you with
- Support as you readjust to life after military service
- Individualized care, specific to your unique post-combat readjustment needs
- Referrals for mental health services
- Information on VA and community services
- Personal assistance navigating VA health care
